The Essessa platform uses configurable assessment forms and present them to users securely over their mobile devices. The forms are designed to acquire medically relevant information from the user which can then be shared with the inquirer and to be responsive to the information given. 

 

In most cases the user is the patient and the information is then shared with a care provider, or an industry partner that is directly involved in the patient's care.  Through this open communication platform, several key initiatives and goals for improving patient communication and outcomes can be accomplished as shown in the diagram.
